Job Description:

To assist in the daily operations of the Howard County Health Department (HCHD) providing direct nursing care to patients in various community settings and HCHD clinics, primarily in reproductive health and family planning. The work may be performed at patients’ homes, schools and other community settings to provide community health nursing services to individuals, families and the community.

Job Responsibilities:

  • Provides client centered comprehensive contraception counseling with a focus on engaging clients in reproductive life planning;
  • Participates in the coordination of communicable disease investigation and surveillance;
  • Records health status of patients, treatments administered, and other appropriate information in the patients’ electronic health record, and reports to the clinician, and to the nursing supervisor as indicated;
  • Participates in the orientation of staff;
  • Participates in ongoing continuing education with a particular focus on reproductive health an family planning.
  • Participates in an on-call rotation for afterhours call;
  • Coordinates the care of a selected caseload of patients for referred services;
  • Participates in identifying community health needs and developing alternative solutions using health department and community resources;
  • Provides nursing care to individuals and families in the HCHD health center/clinics;
  • Works to support the planning, coordination and evaluation of Medical Counter Measure Dispensing and Shelter Operations to facilitate emergency preparedness capabilities;
  • Evaluates the effectiveness of nursing interventions and recommends revisions as necessary;
  • Provides assistance with management of communicable disease outbreaks as additional staff needs dictate;
  • Provides individual assessments of social determinants of health and makes appropriate referrals to a variety of community resources;
  • Performs phlebotomy;
  • Works with professionals from other disciplines to plan and implement healthcare services in schools, clinics and other settings;
  • Collaborates on special projects in the community to promote wellness;

Job Requirements:

  • Experience with Phlebotomy
  • Employees in this classification may be assigned duties which require the operation of a motor vehicle. Employees assigned such duties will be required to possess a motor vehicle operator’s license valid in the State of Maryland.
  • Experience providing HIV testing and Counseling
  • Experience in Reproductive Health (STI testing/Counseling, Family Planning)
  • Bilingual in Spanish
