Job Description:

There will be responsibility for support of all programs that includes but not limited to implementing and coordinating services according to state contract requirements. Responsible for reviewing the state AMPM and HP Contracts to ensure requirements are being met in the program.

Job Responsibilities:

• Provides explanations and information to others on topics within area of expertise
• Supports RN to lead MOC 2.0 Pod rounds – taking lead on completing round template
• Prioritizes and organizes own work to meet deadlines
• First point of contact for questions for CHW within MOC 2.0 Pod
• Investigates non-standard requests and problems, with some assistance from others
• Uses pertinent data and facts to identify and solve a range of problems within area of expertise
• Support the additional training and coaching needed for CHWs
• Act as a senior SME for non-RN level activities
• Identified SME to support CHW in assessment and Plan of Care questions
• Support GL24 FTEs to conduct more field-based care interventions
• Supports MOC 2.0 Pod huddles
• Provides explanations and interpretations within area of expertise
• Analyzes and investigates QOC issues identified within MOC 2.0 Pod

Job Requirements:

• Demonstrated ability to assist with focusing activities toward a strategic direction as well as develop tactical plans, drive performance and achieve targets
• 3+ years of Clinical Experience Minimum Educational Background
• Undergraduate Medical/Behavioral degree or LPN or clinical non-4 year degree
• CHW certification
• Experience working in Medicaid and Medicare health care and insurance industry, including regulatory and compliance requirements
• Intermediate proficiency with software applications Microsoft Word, Microsoft Excel, Microsoft PowerPoint
• Problem solving skills; the ability to systematically analyze problems, draw relevant conclusions and devise appropriate courses of action
• Experience in a behavioral health setting (inpatient, outpatient, residential, community mental health, etc.)
• Excellent verbal and written communication skills; ability to speak clearly and concisely, conveying complex or technical information in a manner that others can understand, as well as ability to understand and interpret complex information from others
